This Privacy Policy explains how we collect, use, disclose, and safeguard information when you visit www.patriot-ems.com, contact us by phone or email, request transport services, apply for employment, or otherwise interact with us outside of a clinical care relationship.
This Privacy Policy applies to personal information collected through our website and general business operations. Protected Health Information (PHI) collected in the course of providing medical transport services is governed separately by our HIPAA Notice of Privacy Practices, which is available on our website and on request.

2.1 Information You Provide Directly
We collect information you provide when you fill out a form on our website, send us an email, call us, request a quote, apply for a job, or otherwise communicate with us. This information may include your name, phone number, email address, mailing address, the name and location of the facility you represent, job title, information about a patient or family member you are seeking transport for (limited to what is necessary to route your request), resume and employment history if you apply for a position, and any other information you choose to share in a message or form field.
2.2 Information Collected Automatically
When you visit our website, certain information is collected automatically through Google Analytics and standard web server logs. This information may include your IP address, browser type and version, device type, operating system, the pages you visit on our site, the date and time of your visit, the referring website, and general location information based on IP address. We do not use additional advertising or behavioral tracking technologies on our website.
2.3 Information From Third Parties
We may receive information about you from third parties in limited circumstances, including from healthcare facilities that refer transport requests to us, from job boards and recruiting platforms where you submit an application, and from background check and motor vehicle report providers during the hiring process.

We may disclose information when required by law, in response to a subpoena or court order, to cooperate with law enforcement or regulators, to protect the rights, property, or safety of Patriot EMS, our employees, or others, or in connection with an investigation of fraud or misconduct.
In a business transaction. If Patriot EMS is involved in a merger, acquisition, reorganization, financing, or sale of all or a portion of its business or assets, personal information may be transferred as part of that transaction.
With your consent. We may share information with additional third parties if you direct us to or give us permission to do so.

Patient care records are retained in accordance with Georgia law and federal recordkeeping requirements applicable to EMS providers.

Access and correction. You may contact us to ask what information we hold about you and to request corrections to inaccurate information. Requests involving PHI are handled under our HIPAA Notice of Privacy Practices.
Do Not Track signals. Our website does not currently respond to Do Not Track browser signals. We treat all visitors consistently regardless of Do Not Track settings.
Job applicants. You may contact the hiring email address listed on our Careers page to inquire about or request deletion of application materials you have submitted.
Depending on your state of residence, you may have additional rights under applicable privacy laws, including the right to request access, correction, or deletion of your personal information. To exercise any of these rights, contact us using the information in Section 12.
10. Children's Privacy
Our website and services are not directed to children under the age of 13, and we do not knowingly collect personal information from children under 13 through the website.
